ABOUT THE LEAP ALLIANCE PROGRAM FOR HEALTH PROFESSIONALS
The Lower Extremity Amputation Prevention (LEAP) Alliance supports interdisciplinary programs for podiatrists and other health professionals who diagnose and manage diabetic foot problems.  Programs include diabetes footcare continuing education programs, the development of practice guidelines, encouragement of research, and the creation of diabetes education programs that enable the early detection and prompt treatment of potentially hazardous foot problems.
Member Programs
•“Find a Bare Your Soles Free Screening Location” Website 
The LEAP Alliance website enables diabetic consumers to obtain information about each foot health professional’s office/facility screening location, screening times, and details about the professional’s practice

• Printed Directory of Free Foot Screening Locations – 
The directory is distributed to diabetes support groups, diabetes educators, service organizations, state agencies, physicians, healthcare facilities and others who may refer diabetic persons to a location that provides free diabetes foot screening

• Training for Personnel Providing Screenings

• Physician Patient Referral Forms to Obtain Screening and Diabetic Footcare – 
The forms are distributed to individual referring physicians through hospital staff activities and personal delivery.  Physicians may refer patients to a specific screening location or provide patients with a printed directory listing all locations and the contact information for each location. 

• MD/DO Physician Neuropathy Alert Program – 
The program includes patient education signs for physician and facility waiting rooms that list the symptoms of diabetic peripheral neuropathy and encourage patients to tell their physicians if they have any symptom. The program sign kit includes Patient Referral Forms and printed directories for referred patients listing all locations and the contact information for each location.

Foot screenings are brief foot exams for adult persons with Type 2 Diabetes to identify circulatory problems, diabetic neuropathy, and other conditions that can lead to severe lower extremity complications and amputations.
